If you have registered or have already attended one of the following:

Marital Education (Relationship Enrichment Series, or Practical Application of Intimate Relationship Skills Training - PAIRS), Premarital Education or Parenting Education (Systematic Training for Effective Parenting - STEP)

You hereby acknowledge your voluntary participation in the above listed class, individual education session, retreat, series, or workshop provided by Samaritan Counseling Centers. ‘Voluntary’ indicates that you have in no way been mandated, forced, or coerced to participate in this educational forum and as such are a willing participant.

Additionally, you understand that Samaritan Counseling Centers, which values confidentiality and makes every attempt to respect privacy, cannot guarantee confidentiality since other participants may unintentionally, or intentionally, reference this training, including those who participate.

Furthermore, you are informed that Samaritan Counseling Centers has made every attempt to ensure that the marital information provided during the above listed class, individual education session, retreat, series, or workshop has been obtained from reliable, research-based sources. Samaritan Counseling Centers is not responsible for any errors, omissions, delays in, or for results obtained from the use of the information. In no event will Samaritan Counseling Centers and its educators, or related partners, agents, or employees, thereof be liable for any action taken, or reliance on the marital information provided.

Finally, you understand that the above listed class, individual education session, retreat, series, or workshop is subsidized by the Federal Government through the Administration for Children and Families, and that as such, no information regarding a specific faith, creed, or belief system will be taught during the hours of such educational forum. While participants may reference their own faith, creed, or belief system during the class, you understand that the information provided is based on current marital research and with no specific doctrine.
